If this will be your first appointment, please visit my client portal here to complete your initial paperwork. Unless I tell you otherwise, you only need to complete the forms under “New Client Paperwork.”
I require a card on file for payment of services, and payment is due before the appointment unless we have made alternate arrangements collaboratively. You will receive a text message from Ivy Pay at the phone number used for booking inviting you to put your card on file. You can use an FSA, HSA or other card for services. Your information will be held confidentially – I won’t know your card information either.
On the day of your appointment, please ensure you have a private space where you will not be disturbed. Some clients appreciate gathering comfort items for the session – tissue, something to drink, perhaps a favorite blanket. Be sure your device is charged, and you may wish to use headphones for added privacy. Please be advised that Google Meet can use a high amount of data. Depending on your cell phone plan, accessing over wifi may be preferable.
